Service coordinator team
Our Service Coordinators act as the bridge between our clients with complex needs and the many services they engage with.
The team provides a wide range of support to improve the lives of people with multiple disadvantage.
They coordinate services, broker support and advocate on our clients’ behalf.
They empower individuals to take control of their own recovery journey.
They establish partnerships with services and agencies to share risk and accountability and find new ways of working.
“Without your help I would never have been able to work with the services that have helped me over the past few years.”

Identifying what works and what doesn't
A key part of a Service Coordinator’s role is to identify and reduce the blocks and barriers that clients experience when attempting to access services. The coordinators record their finding and track good practice. This then provides an evidence base for our system change work.
